One of our client's employees was in the hospital with a serious condition. Our clients TPA paid the claim. We reviewed the claim two years later and discovered the employee's wife's past employer should have paid the claim. We made a substantial recovery from her past employer and returned the money to our client.In another situation, an employee was in the hospital with a serious condition that should have been paid under a three-tiered per diem rate. The TPA paid the charge as a single per diem rate. The TPA still said we were wrong and felt the payment was correct. We went to the provider and after much discussion they agreed with us. We returned a large refund to our client.
